I feel like it is up to the doctor. When I asked my physician for a referral to rehab, he gave me the referral but asked that I find a different doctor. It didn't bother me, it is his business to run as he sees fit. Maybe he has a family member struggling with addiction and he is just fed up with us, I have no idea. He was very kind, polite and professional about it and I went on my way.
But really, would I want someone to treat me who doesn not want to? Nope.

Furniture from Goodwill really skeeves me out. I have purchased clothes there before, but I always wash them twice before wearing (first a soak, then a hot wash). You can steam clean furniture, but that does not get everything. Yuck!
